Men’s Hormone Optimization · The Formulary

Why we inject under the skin.

Most men picture testosterone as a deep, weekly shot into the muscle. It doesn’t have to be. We prefer the subcutaneous route — a small insulin-style needle into the fat of the belly or thigh, twice a week. Smaller doses more often keep your levels steady instead of peaking after the shot and sagging before the next one, which is where most of the mood swings, energy dips and estrogen side effects come from. It’s also nearly painless and easy to do yourself. For men who can’t tolerate the standard cottonseed-oil base, we also offer testosterone in MCT oil. Intramuscular dosing and a daily cream are both available when they suit you better.

Treatments

Replace it, or restart it.

Two different answers to a low level. Testosterone replaces what you are no longer making — the fastest and most reliable route, in three forms. Enclomiphene works the other way round, prompting your own testes to make more, which keeps fertility intact. Your labs, your symptoms and your plans decide which one fits.

Build your plan

Testosterone Cypionate — Subcutaneous

Our preferred route

Twice-weekly injection · 200 mg/mL · up to 200 mg per week

Small doses under the skin, twice a week. Steadier levels, fewer side effects, a tiny needle, and no trip to a clinic. Available in standard oil or MCT oil for men who react to cottonseed or grapeseed bases.

Testosterone Cypionate — Intramuscular

Injection every 1–2 weeks · 200 mg/mL · up to 200 mg per week

The classic route, for men who prefer fewer injections or already know it works for them. Larger doses less often mean more of a peak-and-trough; your physician may shorten the interval to smooth it.

Testosterone Cream

Daily · 200 mg/mL, 100 mg applied each morning

No needles at all. Applied once a day; absorption varies between men, so labs guide the dose. Keep it off skin that touches partners or children until it’s dry.

Enclomiphene

Daily capsule · 12.5 or 25 mg · or the Elevate T all-in-one capsule

Blocks estrogen’s feedback at the pituitary so your body raises its own LH, FSH and testosterone. Preserves fertility and testicular function because nothing is being replaced. Levels rise over weeks, not days. Elevate T is the same 25 mg enclomiphene with 25 mg DHEA, 25 mg pregnenolone and zinc built into one daily capsule.

Support & supplements

Keep what you have.

Replacement quiets your own production, so these run alongside it rather than instead of it. HCG and gonadorelin keep the testes working and fertility on the table — far easier to maintain than to recover. DHEA and pregnenolone sit further upstream as precursor hormones: supplements, not prescriptions, for men who want to support their own production.

Build your plan

HCG (Human Chorionic Gonadotropin)

500 IU twice weekly · 10,000 IU vial, about 10 weeks per vial

Mimics LH, the signal that tells the testes to keep making testosterone and sperm. Used alongside replacement to prevent testicular shrinkage and preserve fertility, or on its own in select cases.

Gonadorelin

Once to twice weekly injection · 100 mcg

A short peptide that prompts the pituitary to release its own LH and FSH — one step upstream of HCG. Gentler and shorter-acting; reconstituted vial, kept refrigerated.

DHEA + Pregnenolone

Supplement

One capsule daily · 90 or 180 count

Precursor hormones that decline with age. Modest, natural support for your own production — not a replacement, and not a prescription. DHEA alone at 25 mg or 50 mg, or paired with pregnenolone at 10 / 25 mg when mood and mental clarity are the priority. Ninety capsules is a three-month supply, a hundred and eighty is six. DHEA is banned in competition by WADA and the NCAA.

  • DHEA 25 mg 90 capsules, $179 — $59.67 a month. 180 capsules, $349 — $58.17 a month.
  • DHEA 50 mg 90 capsules, $229 — $76.33 a month. 180 capsules, $449 — $74.83 a month.
  • DHEA / Pregnenolone 10 / 25 mg priced with the 50 mg option — $229 for 90 capsules, $449 for 180.

Labs

Measured, not guessed.

Testosterone and enclomiphene are prescribed from numbers, not from how you describe your week. That means a panel before you start and a repeat once treatment has had time to settle — then only as often as your results and your health actually require.

Build your plan

Getting them drawn

Your own results, or we order them

If you have had the right panel drawn in the last six months, upload it at intake and your physician will work from that — there is nothing to repeat and nothing to pay. If not, we send a requisition in your name. Most patients walk into a patient service center; a home draw through Quest is available in some areas.

  • Your own Drawn within the last six months, uploaded at intake. No charge.
  • Walk-in Requisition sent in your name, drawn at a Quest or LabCorp patient service center. Roughly $70–$80, paid to the lab.
  • Home draw A phlebotomist comes to you, through Quest, where that service is offered. Priced separately by the lab.

How often they repeat

Once treatment settles, then by what changes

Your first repeat falls within the first five months of treatment, once a steady dose has had time to show up in your bloodwork. After that the schedule follows your situation rather than a calendar for its own sake.

  • Nothing changing Same dose, same health history, results where your physician wants them — annually.
  • Something changed A dose adjustment or a change in your medical history — every five months, or sooner.
  • Urgent New symptoms or an urgent medical issue — whenever your physician needs them.

The rules

What applies to every plan.

Labs come first

Testosterone treatment starts with bloodwork — total and free testosterone, estradiol, blood count, PSA — and is re-checked within the first five months. Upload your own from the last six months, or we order it and you pay the lab directly.

Testosterone is a controlled substance

Schedule III. It ships from a DEA-registered pharmacy after an identity check and a physician’s review, and it cannot be refilled early.

Protect what you have

Replacement quiets your own production. If fertility or testicular size matters to you, HCG or gonadorelin runs alongside it from the start — it’s much easier to keep than to recover.

Estrogen is managed, not feared

Some testosterone converts to estradiol, and a little is healthy. We monitor it and adjust dose or timing first; an aromatase inhibitor is a last resort, not a default.

Know what you’re taking, and why.

Build your plan, complete a short intake reviewed by a licensed physician, and if treatment is appropriate, your plan ships in the supply you chose.
