What we collect, and what we will not do with it.
This policy covers the information Reclaim collects through this website and in running the practice. Your medical record is covered separately and more strictly, by our Notice of Privacy Practices — read that one too.
Summary
The short version.
The rest of this page is the detail. These four lines are the substance.
We do not sell your information. Not to data brokers, not to advertisers, not to anyone.
We do not share your health information with advertisers, and we do not use it to target advertising to you anywhere.
Your medical record is used to treat you, to take payment, and to run the practice — and otherwise only with your permission or where the law requires it.
You can ask us what we hold, get a copy, and ask us to correct it. Contact details are at the bottom.
01
What we collect.
Three kinds of information, held differently.
What you give us
- Your name, date of birth, address, email and phone number.
- Your medical history, medications, allergies and the answers in your intake form, together with anything you upload — laboratory results, photographs, a photo of your ID.
- Messages you send your physician.
- Payment details. We never see or store your full card number — our payment processor handles it and gives us only a token and the last four digits.
What we create
- Your medical record: your physician’s assessment, prescriptions, decisions and notes.
- Your order history, shipment tracking and payment status.
What the website records automatically
- Your IP address, browser and device type, the pages you visit and the times you visit them. This is ordinary server logging and it is what lets us keep the site working and secure.
- Cookies, described under Cookies and tracking below.
What we do not collect
- We do not ask for your Social Security number.
- We do not buy information about you from data brokers.
- We do not knowingly collect anything from anyone under 18. Reclaim treats adults only. If you believe a minor has given us information, tell us and we will delete it.
02
Why we use it.
To treat you, to charge you correctly, and to run a practice. Not for anything else.
- To provide your care — evaluating your request, prescribing, monitoring you, answering your messages.
- To fill and ship your prescription, which means giving the dispensing pharmacy what it needs.
- To take payment and to deal with refunds and disputes.
- To contact you about your care and your orders, by email, text message and phone. What you agreed to is set out in your consent.
- To keep records the law requires us to keep, including prescription records and, for controlled substances, reporting to state prescription monitoring programs.
- To keep the site and the practice secure, and to investigate fraud or misuse.
- To improve the service — which means looking at how pages are used, not at your medical record.
We do not use your health information to decide what advertising you see, and we do not let anyone else do so either.
04
Cookies and tracking.
What we run, and the one thing we deliberately do not.
Cookies we use
- Essential cookies, which keep you signed in and remember your session. The site does not work without them.
- Local storage on your own device, which is how a plan you build on this site is remembered while you browse. It stays on your device and expires after seven days.
- Analytics, where enabled, so we can see which pages people read and where they get stuck. Configured so that it does not receive your medical information.
The one we are careful about
We do not place advertising or social-media tracking pixels on pages where you describe your symptoms, build a plan, or complete an intake. Those pages reveal why you came here, and that is not information an advertising network should receive. This is deliberate, and it is one of the few places where this industry routinely gets it wrong.
Your browser
You can block or delete cookies in your browser settings, and essential cookies aside, the site will still work. We honour Global Privacy Control signals where your browser sends them.
05
How long we keep it.
Medical records longer than you might expect, because we are required to.
- Your medical record is kept for the period the law in your state requires, which is generally several years after your last visit and longer for certain records. We cannot delete it on request before then, even if you ask us to close your account.
- Prescription and controlled substance records are kept for the period federal and state law require.
- Payment records are kept as long as tax and accounting rules require.
- Website logs and analytics are kept for a short period and then deleted or aggregated.
06
Your rights.
Ask us and we will do it, or tell you plainly why we cannot.
- See it and get a copy. Your medical record, in a form you can use.
- Ask us to correct it if something is wrong.
- Ask how we have shared it, and with whom.
- Ask us to communicate with you in a particular way — by portal message only, for example, rather than email or text.
- Ask us to restrict a disclosure. We will agree where we lawfully can.
- Withdraw a consent you gave, including consent to telehealth or to being texted.
- Complain, to us or to a regulator, without it affecting your care.
Your rights over your medical record specifically, and how to exercise them, are set out in the Notice of Privacy Practices.
State privacy rights
Depending on where you live, you may have additional rights over non-medical personal information — to know what we hold, to have it deleted, to correct it, to receive a portable copy, and to opt out of its sale or of targeted advertising. Since we do not sell personal information and do not use it for targeted advertising, there is nothing to opt out of; but the other rights are real and we will honour them. Ask using the contact details below. We will not treat you differently for asking.
Some states also give you the right to appeal if we refuse a request. If we refuse one, we will tell you how to appeal.
07
Security, and its limits.
What we do, and what we cannot promise.
We use encryption in transit and at rest, access controls so that staff see only what their job requires, and systems built for medical information. Our vendors are contractually bound to equivalent standards.
No electronic system is completely secure, and we will not pretend otherwise. Email and text message in particular are less private than messages inside your account; if you would rather we used only the portal, tell us and we will. If a breach affecting your information occurs, we will notify you as the law requires.
People with access to your phone, computer or email may be able to see information about your care. Log out when you have finished, and keep your devices and your password secure. Tell us immediately if you think someone has got into your account.
08
Changes, and how to reach us.
We will tell you rather than quietly repost.
We may update this policy. If we make a change that materially affects your rights, we will tell you directly — by email or in your account — rather than reposting it and treating your next visit as acceptance. The date at the top of this page is when it last changed.
Contact us
This website and the practice are operated by Fort Pitt Telemedicine, PLLC. Our full address and contact details appear at the foot of every page.
For anything about your care, message your physician in your account — it is faster and it goes into your record. For anything about privacy, including a request to see, correct or delete information, email us or call (412) 204-6214.
If you are having a medical emergency, call 911. If you are having thoughts of suicide or self-harm, call or text 988.
This policy covers information collected through this website and in the course of running the practice. Your medical record is governed by the Notice of Privacy Practices, which takes precedence where the two differ.
Nothing on this page is medical or legal advice.
