State notices · Last updated 19 October 2026
Where we can treat you, and what your state requires.
Reclaim is licensed in 22 states and prescribes testosterone in 16 of them. Some states require us to give you particular information, in particular words. Those notices are reproduced here exactly as the state writes them, along with how to raise a concern about a physician wherever you live.
Availability
Where we practise.
You must be physically located in one of these states at the time you are treated.
We are currently licensed in Arizona, Colorado, Florida, Georgia, Indiana, Kentucky, Maryland, Michigan, Minnesota, Missouri, Nebraska, Nevada, Ohio, Oklahoma, Oregon, Pennsylvania, Tennessee, Texas, Utah, Virginia, Washington and Wisconsin.
Testosterone is available in 16 of these states. It is a Schedule III controlled substance and is dispensed only where our state registration is in place. It is not available in Indiana, Maryland, Missouri, Nevada, Oklahoma or Utah, where the other hormone options remain available.
The prescription tretinoin range is not dispensed in Minnesota or Michigan. An alternative is offered in those states.
If you live outside the states we serve, we will tell you before you complete an intake, and nothing is charged. If you move, tell us before you order again — we may not be able to continue your treatment in your new state, and we would rather tell you than have an order fail after you have paid.
Florida
Weight-Loss Consumer Bill of Rights.
Required by Florida law for anyone offering a weight-loss program in the state. The wording below is the state’s, not ours.
Weight-Loss Consumer Bill of Rights
Warning: rapid weight loss may cause serious health problems. Rapid weight loss is weight loss of more than 1½ pounds to 2 pounds per week, or weight loss of more than 1 percent of body weight per week after the second week of participation in a weight-loss program. Consult your personal physician before starting any weight-loss program.
Only permanent lifestyle changes, such as making healthful food choices and increasing physical activity, promote long-term weight loss.
Qualifications of your weight loss provider are available upon request.
You have a right to:
- Ask questions about the potential health risks of this program and its nutritional content, psychological support, and educational components.
- Receive an itemized statement of the actual or estimated price of the weight-loss program, including extra products, services, supplements, examinations, and laboratory tests.
- Know the actual or estimated duration of the program.
- Know the name, address, and qualifications of the person who has reviewed and approved the weight loss program according to section 468.505(1)(j), Florida Statutes.
Our prices, what each one includes, and how long each program runs are set out on How Reclaim works and on the page for each treatment. To ask about the qualifications of your physician, or for an itemised statement, contact us using the details at the foot of this page.

Nebraska
Before your first consultation.
Nebraska law requires that you be given these four points in writing before your first telehealth consultation.
Your rights under Nebraska law
- You have the right to refuse this telehealth consultation at any time. Refusing will not affect your right to future care or treatment, and will not cause you to lose any benefit you would otherwise be entitled to.
- The confidentiality protections that apply to your medical care apply to this consultation.
- You have the right to access all medical information generated during this consultation, under the law governing access to medical records.
- No identifiable images or information from this consultation will be given to researchers or any other entity without your written consent.
If your address is in Nebraska, these four points are also shown inside the intake form itself, before any medical questions, and you will be asked to acknowledge them there.
Utah
What we must tell you first.
Utah requires seven specific disclosures before your first telehealth visit.
Utah disclosures
- Fees. Reclaim charges no separate telehealth fee. What each price includes is set out on How Reclaim works.
- Who may receive your health information, and why, is set out in our Notice of Privacy Practices. We will ask your consent before sending identifiable information to anyone outside your care.
- Your rights over your health information are set out in the same notice.
- What this service is for, and its limits. Reclaim is not for emergencies. If you think you are having one, call 911 or go to the nearest emergency department. If you are having thoughts of suicide or self-harm, call or text 988.
- Security. Our systems meet industry security and privacy standards. Some risk to privacy remains in any electronic transmission of information, regardless of the measures taken.
- Technical failure. Information may be lost or delayed because of a technical failure. Our agreement sets out the limits of our responsibility where that happens despite reasonable security measures being in place.
- Who operates this site. This website is operated by Reclaim, whose legal name, address and contact details appear at the foot of every page.
Every state
If you have a concern about a physician.
You can raise it with us at any time, and you can always raise it with your state medical board instead. Nothing in our agreement prevents that.
A national directory of every state medical board is maintained by the Federation of State Medical Boards at fsmb.org.
This page sets out notices required by particular states. It is not medical advice and it does not replace the Reclaim General Consent and Agreement, which you accept when you submit an intake.
Where a state prescribes the wording of a notice, it appears here exactly as the state writes it. If you believe a notice on this page is out of date, please tell us.
Available in 22 states at launch, testosterone in 16 of them. Testosterone is a controlled substance and is prescribed only where our state registration is in place. If you are outside the states we serve we will tell you before you complete an intake, and nothing is charged.
